CREDIT CARD ON FILE
At the time of check-in, the front desk will request your credit card information. Your credit card numbers will be encrypted and stored securely off-site. We will keep your credit card on file for future visits and balances owed.
No credit card numbers will be stored at our practice.
We will continue to file insurance on your behalf as a courtesy. Co-pays must be paid at the time of the visit. Once we receive your Explanation of Benefits (EOB) (what the insurance company will pay towards your visit), we will balance bill your card for balances less than $100. Any balances owed over $100 after insurance has paid will receive a call, text or email from our office prior to charging to your credit card on file.
